How We Get You Back To Full Strength: The Four Phases Of Recovery At Darwin Health Group

A lot of people who come to see us at our Stuart Park clinic in Darwin tell a similar story. They hurt their back, knee or shoulder, had a few treatments, felt better and stopped. A few months later the same pain was back, usually right when life got busy again.

It is a frustrating cycle, and there is often a simple reason behind it. Feeling better and being better are not the same thing. Pain can settle within days or weeks, but the strength and capacity your body lost along the way take longer to rebuild. When that part is skipped, the underlying problem is still there, waiting for the next big weekend, heavy shift or long drive.

That is why our physios guide people through four phases of recovery. It all starts with a thorough assessment, where we work out what is driving your pain, how you are moving, and most importantly, what you want to get back to. Your goals shape everything that follows.


Phase 1: Calm It Down

When you are in pain, everything else takes a back seat. Sleep suffers, work gets harder and even simple movements can feel risky. The first job is to settle things down.

In this phase we use hands on treatment to ease pain and restore movement, along with gentle exercise and practical advice on what to change while things settle. For most injuries that does not mean stopping everything. Complete rest can leave you stiffer and weaker, so we help you find the amount of activity your body can handle right now.

We also take the time to explain what is going on. Understanding your pain can take a lot of the worry out of it, which makes it easier to keep moving.

Hands on treatment is a useful way to take the edge off and get you feeling human again. On its own, though, it rarely solves the underlying problem. Its real value is that it opens the door to the work that does.


Phase 2: Build It Back

Once your symptoms settle, we start loading the injured area progressively to rebuild strength and capacity. This is the phase most people skip, and it is one of the most common reasons pain keeps coming back.

After an injury, the muscles, tendons and joints around the area lose strength and tolerance for load, sometimes surprisingly quickly. The pain may have eased, but those tissues are not yet ready for the full demands of your life. Muscles and tendons adapt to load gradually, over weeks and months rather than days, so rebuilding them takes consistent work and a clear plan.

Your program is built around your injury, your goals and your starting point. We begin at a level you can manage, then increase the load, volume and difficulty as your body adapts. Sessions in this phase look less like lying on a treatment table and more like a training session, with your physio coaching your technique and adjusting the plan as you go. You will also have exercises to do between appointments, because that is where a lot of the change happens.

It is not always glamorous, but this phase is often the difference between a quick fix and a lasting result.


Phase 3: Return To Life

Getting stronger only counts if it carries over to the things you actually care about. In this phase we gradually reintroduce the activities, sport or work demands that matter to you.

For a footy player, that might mean progressing from running in straight lines to sprinting, changing direction and taking contact. For a tradie, it could mean lifting, carrying and working overhead for a full shift. For someone else it might be getting back out on the boat, keeping up with the grandkids or enjoying a long walk without thinking twice.

We break those demands down into manageable steps and build up in a controlled way. Progress is guided by how your body responds and whether you are meeting clear markers of readiness, not just by how many weeks have passed.

Confidence matters just as much as strength here. Many people are physically ready before they feel ready, and that hesitation can change the way they move. Practising the real demands of your life in a controlled setting means you head back into them feeling prepared rather than nervous. Controlled, confident, and at your pace.


Phase 4: Stay There

Getting you better is one thing. Helping you stay that way is the real goal, so you are not back on the table in six months with the same problem.

Before you finish up with us, we make sure you understand what contributed to the injury in the first place. That might have been a sudden jump in training, a change in work duties, or letting your strength work slide once the pain disappeared. You will leave with a maintenance program you can realistically stick to, and a clear plan for what to do if things start to niggle so a small flare does not turn into a bigger setback.

Some people like to check in with us periodically to stay on track. Others simply want to know they can get in touch early if something does not feel right. Either way, the aim is for you to have the tools and knowledge to look after yourself.


Frequently Asked Questions

How long does it take to get through all four phases?

It depends on your injury, how long it has been going on and what you want to get back to. Some people move out of Phase 1 within a week or two, while rebuilding strength commonly takes several weeks to a few months. Your physio will give you a realistic idea of timeframes after your assessment.

Do the phases always happen in order?

Mostly, but they overlap. We will often start some strengthening while pain is still settling, and your plan adjusts to how you are responding. What matters is that no phase gets skipped.

Do I need a referral to see a physio in Darwin?

No, you can book directly with us without a GP referral. Some funding arrangements, such as Medicare plans, DVA or workers compensation, have their own requirements, so let our team know how your care is being funded when you book.
